Office Policy


Full payment is due at the time of service.  We accept Cash, Check, Debit and Visa, MasterCard, and Discover Card.  We also offer CareCredit which is an extended payment plan prior to credit approval (refer to Financial Policy).

If we are contracted with your insurance company, we request any co-pays, deductibles, and any services not covered by your insurance plan be paid at the time the service is provided.

Late Arrivals

Late arrivals can cause schedule delays for patients who arrived promptly to their appointment. Therefore, if you are more than ten (10) minutes late for your appointment we may need to reschedule you to another day to allow enough time for your treatment.

Cancelation Policy

We treat one patient at a time. As a result, your appointment is specially reserved for you.  If, for any reason, you cannot keep a scheduled appointment or will be delayed, please give us at least 24-hour notice.

Minors – Patients under the age of 18

If the patient is under 18 years old, he or she must be accompanied by a parent or legal guardian.  Payment of services for the treatment is the responsibility of the accompanying adult.
